Same-day appointments are subject to availability.
Clients must provide all laboratory requisitions and valid identification before service.
Cancellations or rescheduling requests must be made at least 24 hours before the appointment.
No-show appointments may be charged up to 100% of the service fee.
Mobile services are available at homes, offices, healthcare facilities, assisted living communities, and workplaces.

Insurance Notice
Lab Life Solutions is a mobile specimen collection service. Collection fees cover the mobile visit and specimen collection process. Laboratory testing fees physician fees, and other provider charges may be billed separately by the ordering healthcare provider, laboratory or Third-Party services provider.

By scheduling an appointment, you acknowledge and agree to Lab Life Solutions’ booking policies, payment terms, cancellation policy, and travel fees, and applicable service charges.
Clients are responsible for providing accurate information and any required laboratory requisition forms before scheduled appointment.
